  1. Follow the instructions listed and click on the “Click Here” to proceed
  2. Click on “Continue as Guest” to start forms.
  3. There are four forms you must fill out: Emergency Information, Insurance, UIL Form, and Athletic Handbook
  4. Each form will be a blue link you can click on to start each form.
  5. Fill in the required information.
  6. Parents and students are required to digitally sign the forms and checkboxes that you acknowledge the information in each form.
  7. Make sure all blanks are filled and all boxes are checked and hit submit to move on to the next page.
  8. Once all four forms are completed the forms will automatically be submitted to the Los Fresnos CISD Athletics staff.
